Level Select
Go to the Sound Test area and play these sounds: 19, 65, 09, and 17. (This is Yuji Naka's birthday, September 17, 1965) You will hear a Ring chime, now head back to the title screen, and hold down the A button and press the start button.
Debug Mode
First activate the level select mode. Now head to the new Sound Test area and play the following sounds: 01, 09, 09, 02, 01, 01, 02, and 04. (November 24, 1992, the same as the US release date) Now, hold the A button down and press the start button on whatever level you wish; when you pause the game press the B button to go in slow motion.
Unlock Zone X (Time Attack)
I guess not much thought was put into unlocking secrets in the game. Basically you must beat the game to unlock this. The four Acts feature some areas that are similar to Sonic the Hedgehog 2. (Da loops man, da loooooops)
Unlock the Super Final Zone (Super Sonic and Super Blaze)
Just like all Sonic the Hedgehog games you got to collect all the Chaos Emeralds with Sonic and finish his story. That's seven Chaos Emeralds for those that aren't following. Now you go back and play as Blaze and finish her story. You don't need to go into any special stages, since Blaze just picks them up after she kicks some ass. Now when you go to select your character you will see a third option with both characters standing next to each other. Now select it and get ready to fight both Dr. Robotnik and Dr. Nega.
